Passenger(s) Details required
Name(s) on all documentation is/are spelled correctly
Name(s) on all documentation is/are traveler’s legal name (as spelled on the VALID government-issued form of ID to be used at the airport)
Passenger(s) date of birth is/are correct (if applicable)
Ensure all names match EXACTLY with the documentation you will use at the airport, at customs and other locations where an ID may be required. Supplier name correction fees are the passenger’s responsibility if Prestige Travel Leaders is not notified within one day of receipt of this form.
Review Your Itinerary
Please review your itinerary to insure accuracy of the information. Here are examples of the information to confirm:
Departure dates and times, flight dates and times, hotel check-in and check-out dates, car rental pick up and drop off dates, special excursion departure and return dates and times, cruise or tour departure and return dates and times, rail departure and arrival dates and times, and each suppliers cancel and change policies.
They may not all apply to you but be sure to review documents for all travelers in your group and double check all of those that do!

Life is unexpected. Things happen and sometimes trips needs to be cancelled or rescheduled. You may get sick or injured on a trip and need medical care. Someone back home gets sick and you need to get home. Travel insurance helps protect you from losses you might incur.
We strongly recommend travel insurance for the protection of your health, your property, baggage, and your trip costs and cancellation fees. Insurance offering various combinations of coverage is available from many sources, including those presented by Travel Machine.
Please note: In some situations (like pre-existing medical conditions) you may be required to purchase travel insurance within a certain amount of time after you purchase your trip. The time varies based on the travel insurance vendor so please ask us for details.
I am aware that all travel expenses will be subject to the loss of all of the fare, or require the expenditure of additional money if I elect to change dates or cancel reservations. I accept that Travel Machine is not responsible for, nor will I attempt to hold liable for any injury, damage or loss I may suffer. If I decline insurance coverage I accept the consequences.*
